Samsung S3 LTE (i9305) : Network PIN prompt ISSUE - please assist

Hi guys,

I really hope that I will find a fix for my issue here as this forum tends to be the best database for android stuff ;)
so details :

Phone : Samsung Galaxy S3 LTE ( i9305 )
Latest ROM : CWM 12
Error : Network PIN

So after using my phone with Android 6 based ROM ( CWM , Ressurection Remix ROM) after one reboot it prompted for Network PIN.
I dug the whole internet for a solution and still have nothing useful.
I have switched different ROMs but after each installation it prompts for Network PIN.
Phone is not locked to any provider.

The IMEI looks good.

Please share any useful advice on this as I cannot use my phone for a month now.
